Here is one example:
I have a macro that contains "cast fireball|t|cast fireball|t|cast fireball|t|cast fireball|t|cast fireball|t". I use this to queue up 5 fireballs at a time.
Here is an example queue output after I've issue my macro above twice and you can see in this example 'cast fireball' twice didn't make the queue:
I see similar results whether it's using an in game macro, a mudlet alias, or simply typing in the commands quicker than my char can use them.Your command queue is:

In situations like above the impact is minor, but there are cases where it's more significant, like my macro "hold mana|drink mana" when times that drink mana doesn't register and I'm left unable to cast again until I realize I'm still holding my mana potion. Or important spells don't trigger (like "cast wall of ice").
It's managable, just a minor annoyance. Wanted to make sure to report it though.
